ABSTRACT

Rectal cancer is the most common tumor of digestive tract. For female patients, ovarian metastasis ranks the second place in intraperitoneal organ metastasis. Its symptoms are occult, easily missed and insensitive to systemic treatment, so the prognosis is poor. Surgery is the treatment of choice for patients with rectal ovarian metastases, whether R0 resection is possible or not, and reducing tumor load is associated with better prognosis. With the continuous development of hyperthermic intraperitoneal chemotherapy (HIPEC), tumor reduction can reach the cellular level, which can significantly improve survival. Prophylactic ovariectomy remains a controversial issue in patients at high risk of ovarian metastasis. In this review, we summarize the diagnosis, treatment and prevention strategies of rectal cancer ovarian metastases, hoping to provide some reference for clinical practice.

ABSTRACT

Objective:To investigate the breakdown of blood-retinal outer barrier in the ischemia-reperfusion injury mice following acute intraocular hypertension.Methods:Fifty-seven SPF male C57BL/6J mice were selected and divided into the control group and high-intraocular pressure (IOP) group by using the random number table method. There were 25 mice in the control group and 32 mice in the high-IOP group. After the failure and poor modeling excluded, 20 mice were included in each group, and the left eyes were selected as the experimental eyes. The ischemia-reperfusion injury model of the high-IOP group was established following acute intraocular hypertension by anterior chamber perfusion of 0.9% sodium chloride solution, and the control group only received anterior chamber puncture.Optical coherence tomography was used to detect retinal thickness.Immunofluorescence staining was utilized to identify zonula occludens-1 (ZO-1) protein distribution in retina.Retinal capillary degeneration was identified by trypsin digestion.Inflammatory cell infiltration in retinal sections was observed by hematoxylin and eosin staining. The use and care of the animals complied with the Statement of the Association for Research in Vision and Ophthalmology, and the study protocol was approved by an Ethics Committee of Changsha Aier Eye Hospital (No.2018-KYPJ005).Results:Compared with the control group, the structure of the retinal pigment epithelium (RPE) layer was irregular with obvious exudation and local neuroepithelial detachment and elevation. The thickness of the full retinal layer of mice in the high-IOP group was (235.8±5.3)μm, which was significantly thicker than (213.3±3.9)μm in the control group ( t=3.427, P=0.009). ZO-1 staining results showed that ZO-1 was mainly located in cell membrane and a small part in cytoplasm in the RPE layer of mice. Two days after modeling, ZO-1 in the high-IOP group was significantly internalized with decrease in cell membrane and increase in cytoplasm, and its distribution was irregular.Seven days after modeling, retinal capillary degeneration was observed in the high-IOP group, and the number of degenerated retinal capillaries was 201.0±13.2, which was significantly larger than 11.2±1.7 in the control group ( t=14.280, P<0.01). Hematoxylin-eosin staining results showed that inflammatory cell infiltration, mainly neutrophils, could be observed in the mice retina with high IOP, and the infiltrated inflammatory cells were mainly located under the internal limiting membrane. Conclusions:Acute intraocular hypertension induced retinal ischemia-reperfusion injury in mice destroys the integrity of the outer retinal barriers, and causes granulocyte infiltration in the peripheral circulation, retinal edema as well as retinal capillary degeneration.

ABSTRACT

Objective:To investigate the curative efficacy of modified Qilang prescription on drug-dependent constipation with Qi and Yin deficiency and the effects on serum vasoactive intestinal peptide (VIP), motilin (MTL), 5-hydroxytryptamine (5-HT), and 5-hydroxytryptamine 4 receptor (5-HT4R). Method:A total of 160 patients diagnosed with drug-dependent constipation were randomly divided into a treatment group (n=80, Qilang prescription) and a control group (n=80, lactulose oral solution). The treatment lasted for eight weeks. Changes in clinical symptoms, traditional Chinese medicine (TCM) syndrome, and serum VIP, MTL, 5-HT, and 5-HT4R before and after treatment were observed. The clinical efficacies of the two groups were compared. An eight-week follow-up was carried out for the observation of recurrent rate and TCM syndrome. Result:The overall response rate of the treatment group (90.91%) was higher than that (75.00%) of the control group (Z=-6.514,P<0.05). There was no significant difference in serum VIP, MTL, 5-HT, and 5-HT4R between the two groups before treatment. After treatment for eight weeks, both groups showed reduced serum VIP level as compared with those before treatment, and the treatment group was inferior to the control group (P<0.05). The serum MTL levels of the two groups were both higher than those before treatment (P<0.05), and the treatment group was superior to the control group (P<0.05). After treatment, the level of 5-HT in the treatment group was higher than that in the control group (P<0.05). The post-treatment 5-HT4R level in the treatment group slightly increased (P<0.05), but no significant difference in 5-HT4R levels between the two groups after treatment was observed. During the eight-week follow-up, the recurrence rate in the treatment group was significantly lower than that in the control group at the 2nd and 4th weeks (P<0.05). There was no significant difference in the recurrence rate between the treatment group [57.14% (40/70)] and the control group [64.81% (35/54)] after eight weeks. Conclusion:Modified Qilang prescription was superior to lactulose in the short- and mid-term efficacy on drug-dependent constipation with Qi and Yin deficiency. No significant difference in the long-term efficacy was observed. The underlying therapeutic mechanism might be related to the regulation of serum VIP, MTL, 5-HT, and 5-HT4R levels.

ABSTRACT

Objective To identify the morphological parameters that are related to intracranial aneurysms (IAs) rupture using a case-control model. Methods A total of 107 patients with multiple IAs and aneurysmal subarachnoid hemorrhage between August 2011 and February 2017 were enrolled in this study. Characteristics of IAs location, shape, neck width, perpendicular height, depth, maximum size, flow angle, parent vessel diameter (PVD), aspect ratio (AR) and size ratio (SR) were evaluated using CT angiography. Multiple logistic regression analysis was used to identify the independent risk factors associated with IAs rupture. Receiver operating characteristic curve analysis was performed on the final model, and the optimal thresholds were obtained. Results IAs located in the internal carotid artery (ICA) was associated with a negative risk of rupture, whereas AR, SR1 (height/PVD) and SR2 (depth/PVD) were associated with increased risk of rupture. When SR was calculated differently, the odds ratio values of these factors were also different. The receiver operating characteristic curve showed that AR, SR1 and SR2 had cut-off values of 1.01, 1.48 and 1.40, respectively. SR3 (maximum size/PVD) was not associated with IAs rupture. Conclusions IAs located in the ICA are associated with a negative risk of rupture, while high AR (>1.01), SR1 (>1.48) or SR2 (>1.40) are risk factors for multiple IAs rupture.

ABSTRACT

<p><b>OBJECTIVE</b>To establish an animal model to replicate the blunt impact brain injury in forensic medicine.</p><p><b>METHODS</b>Twenty-four New Zealand white rabbits were randomly divided into control group (n equal to 4), minor injury group (n equal to 10) and severe injury group (n equal to 10). Based on the BIM-II Horizontal Bio-impact Machine, self-designed iron bar was used to produce blunt brain injury. Two rabbits from each injury group were randomly selected to monitor the change of intracranial pressure (ICP) during the impacting process by pressure microsensors. Six hours after injury, all the rabbits were dissected to observe the injury morphology and underwent routine pathological examination.</p><p><b>RESULTS</b>Varying degrees of nervous system positive signs were observed in all the injured rabbits. Within 6 hours, the mortality rate was 1/10 in the minor injury group and 6/10 in the severe injury group. Morphological changes consisted of different levels of scalp hematoma, skull fracture, epidural hematoma, subdural hematoma, subarachnoid hemo- rrhage and brain injury. At the moment of hitting, the ICP was greater in severe injury group than in mild injury group; and within the same group, the impact side showed positive pressure while the opposite side showed negative pressure.</p><p><b>CONCLUSIONS</b>Under the rigidly-controlled experimental condition, this animal model has a good reproducibility and stable results. Meanwhile, it is able to simulate the morphology of iron strike-induced injury, thus can be used to study the mechanism of blunt head injury in forensic medicine.</p>

ABSTRACT

Under the guidance of the concept of fast tract surgery (FTS), peri-operative managements including pre-operative, intra-operative and post-operative treatments can be optimized to accelerate the rehabilitation of surgery patients. Post-operative recovery of gastrointestinal functions is one of the key links of FTS, in which Chinese medicine has certain features and advantages over it. Current assessments on post-operative gastrointestinal functions recovery include clinical and laboratory assessments, but there is still no industrial assessment received by all. The current situation of post-surgery assessment of peri-operative gastrointestinal functions was studied in this paper, and the research orientation of this field was analyzed and prospected.

ABSTRACT

To describe the imaging features of gastrointestinal stromal tumors [GISTs] at initial presentation with clinical, surgical, and pathologic correlation, and to evaluate values of various techniques in GISTs. This retrospective study recruited 70 patients with histologically proved GISTs between December 2004, and May 2009 in the Department of General Surgery, Zhongshan Hospital, Fudan Univeristy, Shanghai, China. Each patient underwent CT scanning, 39 patients underwent simultaneous endoscopy, 12 patients underwent endoscopic ultrasound [EUS], and 36 patients underwent transabdominal ultrasonography [TAUS] simultaneously. Features of GISTs were assessed. Computerized tomography findings showed an eccentric mass in 44 patients, an intraluminal component in 24, and a transmural distribution in 2. Forty-two tumors were dumbbell-shaped, 2 were round, while 26 were irregular. Forty-three tumors presented with well-defined masses, while 27 with unclear borders. The arterial phase attenuation showed the continuous enhancement. The portal-venous phase attenuation was heterogeneous in 26 and homogeneous in the other 44. There was a significant correlation between certain CT features and tumor risk stratification. Gastrointestinal stromal tumors were characterized by a smooth shape and normal overlying mucosa in endoscopy, hypoechoic, and solid in TAUS. Imaging examinations are pivotal in the management of GISTs. The CT scan is valuable in the diagnosis, staging, and treatment planning of GISTs. Endoscopy and EUS contribute to the detection of mucosal lesions. Other methods including TAUS, fluorodeoxyglucose positron emission tomography, CT gastrography, and MRI help in specific cases

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effect and mechanism of E-leng Capsule (ELC) in preventing and treating post-operation recurrence of ovarian endometriotic cysts (OEC).</p><p><b>METHODS</b>A total of 60 patients enrolled were the inpatients, who underwent cystectomy of OEC in the Gynecological Department of Guangdong Provincial Hospital of Traditional Chinese Medicine from 2002 to 2004. They were randomly assigned to two groups, the treatment group and the control group. Patients in the treatment group were administered with ELC before operation and after operation (starting from the 7th day post-operation), each for 3 months, while patients in the control group were untreated before operation and administered with Danazol from the 7th day post-operation for 3 months. Specimens of eutopic and ectopic endometrium in all patients of both groups were taken during the operation for detecting expressions of matrix metalloproteinase-9 (MMP-9) and tissue inhibitor of metalloproteinase-1 (TIMP-1) mRNA by polymerase chain reaction (PCR), and the incidence of post-operation recurrence was observed 12 months later.</p><p><b>RESULTS</b>The 12-month follow-up showed that recurrence presented in 1 patient in the treatment group (3.3%), and 6 in the control group (20.0%), showing significant difference between them (P<0.05). The expressions of TIMP-1 mRNA in eutopic and ectopic endometrium were 0.85 +/- 0.36 and 0.76 +/- 0.57 in the treatment group, and 0.71 +/- 0.33 and 0.56 +/- 0.39 in the control group, while the expressions of MMP-9 mRNA were 0.36 +/- 0.40 and 0.35 +/- 0.35 vs 0.36 +/- 0.33 and 0.72 +/- 0.59, respectively. The expression of endometrial TIMP-1 mRNA, either eutopic or ectopic, was significantly higher, and the expression of ectopic endometrial MMP-9 mRNA was lower in the treatment group than that in the control group (P<0.01).</p><p><b>CONCLUSION</b>ELC can reduce the invasive ability of endometrium tissue probably by regulating the balance of MMP/TIMP, namely, increase the expression of eutopic and ectopic endometrial TIMP-1 and decrease the expression of ectopic endometrial MMP-9, and thus to achieve its preventive and therapeutic effect on recurrence of OEC.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To demonstrate the changes of resting energy expenditure (REE), substrate metabolism and body composition in cancer patients.</p><p><b>METHODS</b>From September 2004 to March 2008, REE, carbohydrate oxidation (CO) and fat oxidation (FO) in 936 cancer patients and 840 control subjects were measured by indirect calorimetry. Bioelectrical impedance appliance was applied to assess intracellular fluid, extracellular fluid, fat mass (FM) and fat free mass (FFM) in the two groups.</p><p><b>RESULTS</b>No difference in REE was found between the cancer patients and non-cancer patients [(1452.2 +/- 196.4) kcal/d vs. (1429.5 +/- 182.6) kcal/d, P = 0.136]. But REE/FFM and REE/pREE were elevated in cancer patients than in controls (all P < 0.05). Of the cancer patients, 48.6% were hypermetabolic, 42.9% normal and 8.5% hypometabolic, while those were 22.5%, 58.5% and 19.0% in controls. Cancer patients had higher FO [(77.8 +/- 11.3) g/min vs. (67.1 +/- 12.1) g/min, P = 0.000], lower CO and npRQ [(68.7 +/- 10.5) g/min vs. (88.8 +/- 12.1) g/min, P = 0.000; 0.782 +/- 0.012 vs. 0.810 +/- 0.014, P = 0.000]. Cancer patients exhibited lower FM and FFM [(14.9 +/- 4.5) kg vs. (18.4 +/- 5.2) kg, P = 0.000; (44.4 +/- 7.2) kg vs. (46.1 +/- 8.1) kg, P = 0.008].</p><p><b>CONCLUSIONS</b>Elevated REE is common in cancer patients. Substrate metabolism of the cancer patients features in increased FO, decreased CO and npRQ, which is correlated with the elevated REE. FM and FFM loses in proportion in cancer patients.</p>

ABSTRACT

The researches reported in this article are focused on the autofluorescent signal collection of colorectal tissues by laser induced autofluorescent diagnostic system, and on the signal processing by power spectra and networks methods, Pretty good clinical diagnostic sensitivity and specificity are acquired. These may serve as a foundation on which to develop a system for diagnosis of colorecal cancer.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effect of allitridin injection on the expression of human cytomegalovirus (HCMV) immediate-early antigens (IEAs including IE72 and IE86) in human embryonic lung cells.</p><p><b>METHOD</b>HCMV AD 169 Virus strain infected cell model (MOI = 2.5 and 0.25, respectively) were established, and then treated with ICm5 and MTC doses of allitridin. Western blot was used to analyze the of IE72 and IE86 expression after the treatment, ganciclovir(GCV, IC50 and 2.3 x IC50) treatment as control.</p><p><b>RESULT</b>No matter what kind of MOI was used, both IE86 and IE72 antigens' expression was effectively suppressed by allitridin treatment, and the inhibitory rate of IE86 was almost twice of IE72's. Compared with GCV, allitridin had stronger inhibitory effect on IE86 expressing, although its efficacy on IE72 was weaker than GCV.</p><p><b>CONCLUSION</b>Allitridin could suppress the expression of IE72 and IE86, especially for IE86 expressing, maybe it is ore of key role in the mechanism of allitridin against HCMV.</p>
